  一、住院慰问金申请：每位在职工会会员每年限申请一次，需在当年提出申请。系统全部审核完成后，可打印出报销凭证和纸质审批表，由本部门审核人员及本人签字并盖章后，以部门工会为单位送交工会。为减少部门工会工作量，部门工会可于每月10号、20号（遇周末节假日可顺延）集中审核盖章后送交工会纸质材料。<br/>

  二、因生活困难申报“元旦、春节”困难补助（慰问）金的会员，还须同时向部门工会提交下列相关证明：<br/>
|
|
    （1）申请人配偶、子女及家庭其他成员的收入情况证明原件（不涉及则无需提供）；<br/>
|
|
    （2）个人收入证明（年度工资银行对账单且有银行或单位盖章）；<br/>
|
|
    （3）涉及会员本人、配偶或子女患重病、大病者须提供病史证明复印件和自费医药费正式发票；
|
